- Critical: Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed raw ground beef displayed above ready to eat foods and raw chicken displayed above cooked chicken in reach in freezers.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
- Observed improper storage of food items. Numerous food items observed stored directly on the floor in the walk in freezer.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
- A label on a food packaged in the RFE or FSO did not contain all required information. Packages of potato balls in reach in freezer were observed with labels that did not contain the manufacturer, ingredients, and other necessary information
Foods shall be labeled as specified in this rule. Manager immediately removed the potato balls from sale.
- Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed small openings at the base of the door at the delivery dock.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
- Critical: Observed unapproved application or use of a poisonous or toxic substance. Observed a spray bottle containing quaternary sanitizer solution in the deli area. Per labeling instructions, the sanitizer concentration is to be between 150-400 ppm. However, the sanitizer solution was observee at a level that exceeded 400 ppm.
To prevent health hazards, poisonous or toxic materials shall be used and applied according to law and as specified in this rule. Manager immediately discarded the sanitizer.
