- Observed improper storage of food items. -Observed large bags of produce and large mixing bowls of food stored directly on the walk-in cooler floor.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. -During inspection, temporary standoffs were provided and food was moved off the floor. -Discontinue storing items directly on the floor.
- Observed a build-up of dirt and debris. -Observed buildup on the floor, especially behind the cooking equipment, along the walls, under the 3-compartment sink, and on the floor of the walk-in cooler.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. -Clean facility floor to remove buildup.

- Equipment components are not intact, tight or properly adjusted. -Observed ice buildup on the walk-in freezer door due to a torn gasket.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. -Replace gasket and repair freezer door to allow it to close properly.
