To prevent foodborne illness, except for products obtained from a home bakery registered by the Ohio Department of Agriculture or products as specified in 3717.021 of the Ohio Revised Code, food prepared in a private home may not be used or offered for human consumption in a food service operation or retail food establishment.
- Critical: Observed food packages that are not properly labeled provide name of producer, and address.
To properly inform consumers, packaged food shall be labeled as specified in law.

Nonfood-contact surfaces of equipment shall be kept clean. Clean and sanitize interior surface of microwaves to prevent contamination of food.
- The physical facilities are not being maintained in good repair. Observed a large cut in lining of counter top, beneath sandwich presses, allowing for dirt and food debris to be trapped underneath.
The physical facilities shall be maintained in good repair. Resurface counter top to allow for a smooth and easily cleanable surface.

- Equipment and/or components are not maintained in good working order.-remove ice build up from the interior of the freezer.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
- Critical: Utensils and food-contact surfaces of equipment were not sanitized at the required frequency. trays for food and mixers containers.
Utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.
