  • A label on a food packaged in the RFE or FSO did not contain all required information. Observed several re-packaged food items including potato salad, chicken salad, tapioca, and banana pudding, that did not have ingredients listed.
    Foods shall be labeled as specified in this rule. Any individually packaged food for customer self service must contain and ingredient list including the break down of the individual ingredients.
  • Bulk food display for customer self-service was not properly labeled. Donut self service display case did not have an ingredient list readily available in bakery.
    Bulk food that is available for consumer self-dispensing shall be prominently labeled with the information specified under (C)(2) of this rule.
  • The physical facilities are not being maintained in good repair. Observed leaking condesate line in freezer in bakery, prominent ice build up on floor beneath.
    The physical facilities shall be maintained in good repair. Repair leaking condesate line in freezer to prevent possible contamination of foods in freezer.

  • Equipment and/or components are not maintained in good working order. -Observed the condensate line leaking onto packaged products inside the walk-in cooler.
    Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. -Call for repair to eliminate leaking condensate line. -During inspection, food that had been contaminated by water from the leak was voluntarily discarded.
