  • Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container. Observed drink cup with no lid on prep sink table where produce was being prepared for service.
  • Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed drink cup on prep table next to produce that was being prepped for service.
  • Basic - Employee personal items stored with or above food, clean equipment and utensils, or single-service items. Observed a pair of glasses on prep table with breakfast items for service and in use utensils and clean dishes. **Corrected On-Site**

  • High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee finish preparing toast without gloves on and then wipe hands on shirt and return to food prep.
  • High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee remove toast from toaster, place on cutting board, butter and cut toast, then plate it all with no gloves on.
