You also need to worry about age for juvenile labor laws, and having staff who are old enough to sell tobacco and alcohol, but the categories listed aren't specifically geared towards that.
That is easily accounted for by asking something like "Are you 16/18/21/etc. years of age or older?" (and if not then to specify your age/DOB) which would not screen anyone out based on being too old.

I am just coming off a job search (moved to a new area 4 months ago....and really needed to land something so I applied to probably almost 300 job postings). A few times I saw a question along the lines of "Have you reached your 40th birthday yet?" on job applications. It particularly stuck out at me because I just turned 40 yesterday (and got a job offer earlier in the week...thankfully they did NOT ask for my age at any time). I found the question to be very odd.
