I want to say with some of my previous jobs I was contacted by HR probably same day and sent a written offer, but these were also very large companies. I verbally accepted an offer (small local company) over the phone with an annual salary/start date agreed upon on Friday. I was told "You will be sent an offer letter and also have to get a drug test, but that won't be sent until mid week because of the holiday we are celebrating." I took that as offer letter sent same day. I have have not been sent one, and of course have read stories on the internet about companies ghosting people, taking back offers, etc. When should I be concerned?
This isn't 1975. When an offer happens, it is usually by email, frequently a legally signed document in the form of a PDF or a DocuSign that you have to sign, date, and send back.
You're not hired until you have that signed letter in hand, and even then things might fall apart.

Don't give notice or tell anyone anything until you have a signed offer with a start date that confirms you have cleared their background checks and drug testing. Don't contact them again about it.
Don't ever be concerned. Keeping looking for a job, and if by the time they get around to doing something, you can determine if you still think highly of this company and want to work there or not.

Well, emailed them mid last week to follow up, as I already put in vacation time at current job and didn't want to waste it (didn't give notice). No response. I am doing what you said, bc if you don't you have the common courtesy to at least email an update, I take it there is no job offer. Pretty crappy too. I already have 2 interviews lined up for next Friday. I'm also not starting at any place unless I have paperwork in email/hand to verify salary/pto etc..

For years in forums, I see people only focused on the employee not to do anything to "burn bridges", but few seem to worry about the employers behaving this way. To help out others, alert unprofessional behavior like this in websites such as Glassdoor.
We all wish you success with finding a better company with better treatment.
Almost any large organization I've worked for has been at least a week or two, assuming I didn't interview on basically the tail end of their interview process.
