Observation: The quat sanitizer tested at 0 ppm. The unit was remixed to 200 ppm.

Observation: The hand sink did not have a hand wash sign placed by it in the back salsa prep area. I provided a hand wash sign.
- Storage and maintenance of wet and dry wiping cloths
Observation: Common towels were setting on the counter tops after each use.

Observation: Tongs by the bread area were not clean to sight. The units were taken off the shelf to be cleaned.

Observation: The facility did not have test strips to test quat santizer with.

Observation: The facility does not have a certified manager on site.
- Food is properly labeled
Observation: Small loafs of bread were not labeled with an ingredient list, net weight, product name, and address where the product is made.
- The ability to explain the responsibility of the person in charge for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease.
Observation: The person in charge could not find their sick policy or remember the 5 food borne illnesses and the 5 food borne illness symptoms. I handed the owner a sick policy and went over the policy with them.
- Sanitizers
Observation: The bleach water was 200 ppm. The employee remixed the bleach water and it tested 50 ppm.
- Established procedures for responding to vomiting and diarrheal events
Observation: The facility did not have a diarrhea vomit clean up plan. I handed them a diarrhea vomit clean up plan and they stated they would write one of their own..
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Observation: The vent screens in the walk in cooler were not clean to sight.
- Storage and maintenance of wet and dry wiping cloths
Observation: Common towels were being left on the counter tops after each use.
- When to wash
Observation: The meat prep guy was washing his hands then placing old gloves he was previously using. I discussed with the man why using old gloves contaminates hands and is a potential cross contamination risk. He rewashed his hands and changed to new gloves.
