Observation: Vent cover is duty in Maximum reach-in cooler. Clean more frequently to prevent contamination.

Observation: Dish machine was not working, chlorine concentration was tested lower than 50ppm. Called for service. Corrected at time of inspection.

Observation: The following coolers were out of temperature: 1) Milk cooler, food was temped at 44*F 2) Maximum reach-in cooler, food was temped at 44*F. Called for service.

Observation: Several spatulas were in poor condition. All utensils must be maintained in good condition to prevent contamination buildup or bacteria. Corrected.

Observation: Door gasket on milk cooler in dining area is torn. Must replace.
- Package integrity
Observation: Observed dented can of carrots. May not use a can that is dented on top, bottom, or side seals. Also any can that cannot sit level or has sharp points due to denting. Can was discarded.
- Material characteristics of non-food contact surfaces
Observation: Black fiber board on rack by the 3 hole sink is not smooth, easily cleanable, and nonabsorbent. Replace with material that meets all above criteria.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Wire racks by the 3 hole sink and flat top are soiled and rusty. Also black mat under the prep table has become soiled. Racks must be cleaned and resurfaced or replaced, and mat must be cleaned.
- Cleanability of floors, walls, and ceilings
Observation: Floors and walls are in disrepair in the kitchen area, especially in high traffic and moisture areas. Walls and floors must be resurfaced and or replaced to prevent buildup of bacteria and contamination.
