    Observation: Ice machine has buildup on the inside of the bin.
  • Equipment location, installation, repair, and adjustment
    Observation: AdvantEDGE 3 door cooler (on line) has a torn door gasket (Left). Also the white Magic Chef cooler has a broken door.
  • Plumbing system maintained in good repair
    Observation: Drain line for the 3 hole sink (middle and right compartments) has a leak.
  • Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
    Observation: The following areas have become soiled: wall behind the 3 hole sink/dish machine, all wire racks, and the floor drain behind the basement walk in cooler.
  • PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
    Observation: Black GE fridge was temped at 45.5*F. All food was discarded.
  • Miscellaneous sources of contamination
    Observation: White Frigidaire freezer in kitchen has ice buildup inside.
  • Bare hand contact with ready to eat foods
    Observation: Employee was observed handling ready to eat food with bare hands. Food was discarded at time of inspection.
  • Cleanability of floors, walls, and ceilings
    Observation: Kitchen walk in cooler and the kitchen floor are both peeling paint.
  • Material characteristics of non-food contact surfaces
    Observation: Cardboard was being reused in the walk in cooler.
  • A variance request has been submitted and contains required information
    Observation: Facility was pickling and vacuum packaging food on-site. May not use either of these processes unless variance has been applied for. All items discarded.
  • Backflow protection
    air gap, device standard, when required
  • Sanitizers
    Observation: Bar area sanitizer was tested @ 100ppm.
