1) Ice damage has caused walk-in freezer flooring to crack and come up. All flooring installed must be cleanable. Suggest large gauge stainless steel. 2) Lights in the hot window are not shielded or shatter proof. All lights exposed to food and food items must be shielded or shatter proof. Replace bulbs.

Eevry entrance and exit to be used by the public and employees must have state approved "No Smoking" sign posted given during inspection.
- Package integrity
A can of crushed tomatoes had a sizable dent on side. Contents could be contmainated due to compromised package. Discarded. CORRECTED.

Many bottles in establishment were not labeled. When chemicals are removed from the original container they must be labeled with common name. Labeled during inspection. CORRECTED.
- Handwashing cleanser, availability
No soap was available at bar hand sink. Every hand sink must be stocked with soap for proper hand washing. Restocked during inspection. CORRECTED.
- Storage and maintenance of wet and dry wiping cloths
1) Observed several towels laying around kitchen and next to spray bottles of sanitizer. Wiping cloth must be maintained in sanitizer in between uses. 2) Reusing cardboard in the reach-in freezer for bags of portioned apps. All surfaces exposed to food and food items must be nonabsorbent and cleanable. Replace with cleanable containers. 3) Heat tape on walk-in freezer door is not working. Gasket on freezer is torn and in disrepair. All equipment is to be maintained in accordance with manufacturer requirements. Repair heating tape on door and than replace gasket.

The pizza prep cooler was holding cooked hamburger at 47.1 drgees on top and taco meat at 43 degrees in doors. Ambient air temp was at 42.1 degrees. All potentially hazardous foods must be maintained at 41 degrees or less. Cooler turned down during inspection. CORRECTED.

Garnishes in bar cooler were not covered. All food must be covered when not in use to prevent contamination. Cover containers.
