Observation: Light in storage area has cracked shiled. All light bulbs must be shielded, coated, or shatter-resistant in areas where there is exposed food to prevnet physical contamination. Replace cover.

Observation: The receiving / delivery door has light showing throughout the bottom. Outer openings of a food establishment must be protected against the entry of insects and rodents. Replace weather striping door sweep or seal outer edges to prvent entry pests.
- Separation from food, equipment, utensils, linens, and single service
Observation: Chemicals such as "hypoallergenic wipes" stored next to sinlge use utensils and cups on the shelf. Toxic chemicals materials must be stored separated so they can not contaminate food, utensils,and single -use articles. Separate from single use utensils. Corrected on site.
- Handwashing signage
Observation: Hansdwashing sign not posted at prep sink that is designated as hand washing sink now by the office are. Every hand washing sink to be used by employee has sign posted remidning employee to wash their hands.
- Equipment location, installation, repair, and adjustment
Observation: The following gaskets: in reach in coolers (capuchino, juice, milk and arizona) are torn and in disrepair. All equipment must be maintained according to manufacturer requirements. Repair as needed.

- Outer openings are protected
Observation:The delivery back exit door CONTINUES to have light shining through the bottom. THIS IS A REPEAT VIOLATION. All outer doors shall be provided with self closing tight fitting doors to prevent entrance of pests and rodents. Per the manager this door has had weather stripping installed several times. Suggest weatherstripping re-installed or some other means to keep from having any light shining.

- Miscellaneous sources of contamination
Observation: In the reach in freezer two containers holding small cups of flavored coffee ice cream have a excessive amount of ice build, the ice cream cannot be removed due to the excessive amount of ice build up.The ice cream and the containers were removed from the freezer during this inspection. The manager is planning on defrosting the containers and discarding the ice cream. CORRECTED ON SITE.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Both restrooms CONTINUES to have some dirt build up on the ceiling vents. Clean as often as necessary to keep clean. The manager cleaned the vents during this inspection. CORRECTED ON SITE. REPEAT VIOLATION.

Observation: One dented can of chicken noodle soup found in the food establishment. The manager removed from the shelf during this inspection and will send back to distributor. CORRECTED ON SITE.

1. Caulk around the sink in the women's RR, will be smooth and cleanable. 2. Observed ceiling tiles along the liquor coolers are soiled or water damaged. Manager reports the leak was repaired yesterday. Replace soiled ceiling tiles with new ones.
- Drying mops
Mop was lying in the mop sink. After use, mops shall be placed in a postion that allows them to air-dry without soiling walls, equipment or supplies.
- Package integrity
Found one dented can of Hunt's sandwich and box of Tide that was leaking detergent. Discard or return for credit. Manager will return for credit.

1. Noted torn gaskets on left 1st and 4th doors of the beer cooler. Replace as needed. 2. Observed bottom shelving in reach-in milk cooler food residue. Clean frequently. Not clean to sight.
- Plumbing system repaired according to law
Observed in the women's RR that water was leaking around both hot and cold knobs on the faucet. Either have maintenance repair or a plumber. Plumbing must be maintained in good repair.

No handwashing sign in the women's RR. Sign given to manager to post.
- Outer openings are protected
Observed daylight coming in under the back door where deliveries are made. Manager reports that it was just replaced and has to do with the humidity. Monitor and if gets worse, replace with weatherstripping. Outer openings shall be protected from entry of insects or rodents.
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Clean the ceiling vents in both the men's and women's RR and on the Master Bilt reach-in cooler. Not clean to sight or touch.
