Observation: Found some dented cans in the dry storage areas. May not sell any can that is dented on top, bottom, or side seals. Also any can that cannot sit level or has sharp points due to denting. Cans were discarded at time of inspection.

Observation: Cardboard was observed being reused. May not use cardboard for anything but the original product.
- Use of cast iron, wood, or nonstick coatings as a food contact surface
Observation: Found wooden spoon in drawer. May not use wood as a food contact surface unless primarily for baking purposes. Spoon was discarded at time of inspection.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: WindMachine (by dish machine) fan has become soiled and must be cleaned.

Observation: Hand soap was dry in soap dispenser near by ice machine. Replace new hand soap.

Observation: Didn't have sanitizer made up at time of inspection. Must have sanitizer made during all business hours to prevent buildup of bacteria. Corrected at time of inspection.

Observation: Moldy tomatoes stored in walk-in cooler. A food that is unsafe or adulterated must be discarded. Corrected at time of inspection

Observation: Three door reach-in cooler was still out of temperature, the internal was temped between 50.8 to 54*F. No potentially hazardous food in the cooler. Part is on order. May not use cooler for potentially hazardous food until approval has been given through health department.

Observation: Three compartment reach-in cooler was out of temperature, the internal was temped at 51.3*F. Food internal temperature between 48*F to 52*F. Discarded all potentially hazardous food from the cooler. Must repair to ensure it works properly. May not use cooler for potentially hazardous food until approval has been given through health department.

Observation: A small amount of mold is on the front side ice chute in ice machine. Clean more frequently to prevent contamination.

Several boxes of food were stored on the floor under shelving in walk-in freezer. All foods must be 6" off the floor to prevent contamination. REPEAT VIOLATION.

A piece of ceiling tile in employee restroom has a hole. Replace a new tile to prevent entry pest.

1) Shelving liner in dry storage are not cleanable. 2) Reused a cardboard box to store spices. All surfaces exposed to food must be nonabsorbent and eaily cleanable. Box removed during inspection. 3) Sanitizer buckets were being stored on ground in dining area. Sanitizer buckets must be stored off the floor. used turned over crates to coorect the violation.

- Multiuse food contact surfaces are constructed of safe materials
Appears to be a mop sink hose attached to food prep sink faucet used to prep fruit and vegtables. Must use food grade hoses to wash produce. Hose removed during inspection. If replaced ensure a food grade hose is used.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Fan at the end of the dish machine has dust build-up. Clean and maintain cleaning at frequency to prevent contamination of equipment.
- Food storage - preventing contamination from the premises
Several boxes in the freezer ewere on the floor under shelving. All food and food items must be stored a minimum of 6" off the floor. Move product to provided shelving.
- Handwashing cleanser, availability
Soap dispenser provided at hand sink by ice machine was not working. Eevry hand sink must be provided with soap. Repair and/or replace dispenser as needed.
