Observation: Observation: Single use items (cups) stored on the floor in the back storage area. All equipment utensils and single-service items must be stored in a clean, dry location where at least at least 6' inches off the floor. Store on the shelves. Corrects on site. COS.

Observation: The reach-in cooler does not have no thermometer. Provide a thermometer, which is easily seen to verify food is at correct temperatures hold at least 41* F or lower. Provide one.

Observation: Have no food thermometer to check proper cooking of pizza. Must have thermometer to accurately measure the temperature in food such as pizza. Provide one.
- Maintaining premises free of litter and unnecessary equipment
Observation: Unused pieces of equipment- old cooler stored in the back storage area. The premises should be free of non-functioning equipment that is not in use. Remove from food establishment if not in use.

No QA test strips available for sanitizer. Must provide test strips to ensure concentration of QA sanitizer is maintained at 200 ppm. REPEAT VIOLATION.

Handwashing sing not posted at front hand sink. Every sink to be used by employees must has sign posted reminding employees to wash their hands.
