Observation: Excessive ice buildup in chest pizza freezer in back area. Must defrost to prevent source of contamination. Must keep freezer free of ice.

Observation: : Did not have sanitizer solution in 3 compartment sink at time of inspection. Must provide chlorine or QA sanitizer during all business hours and check with the test strips to maintaine correct concentration between 50-100ppm. Corrected on site.
- Equipment location, installation, repair, and adjustment
Observation: Plastic container for popcorn machine has damaged and cracked surface. All food contact surface and equipment need to be maintained in good condition. Replace the plastic.
- Food storage - preventing contamination from the premises
Observation: 1) Dill pickles, 2) juice stored on floor in walk-in cooler, 3) Boxes of single service items stored on floor in dry storage. Owner states that her personal food. Must separate (designate) or and must be stored at least 6" off the floor to prevent contamination.
- Food temperature measuring devices are provided and readily accessible
Observation: No food thermometer to check if pizza is fully cooked. Food temperature measuring device must be be provided and readily accessible for use if food fully cooked. Provide the food thermometer.
- Backflow prevention device located so that it may be serviced and maintained
Observation: Drain line is leaking in a blue bucket from "Glycol machine" anti-freeze drain. A backflow prevention device shall be located so that it may be serviced and maintained, the hose must have at least a 1" air gap. Shall provide at least 1" air gap. The hose was zipped tied and was raised to 1"above sink.

- Handwashing sink equipped to provide water at a temperature of at least 100° F through a mixing valve or combination faucet.
Observation: The handwashing sink had dishes in the sink. A handwashing sink shall be accessible at all times for proper handwashing CORRECTED ON SITE.
- Storage of clean equipment and utensils
Observation: Wrapped paper plates for pizza sitting in the service sink. Shall not have any single service items in the service sink to prevent contamination. CORRECTED ON SITE.
