- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation:There is no detectable level of sanitizer in the rinse cycle - the pump is working but no chemical is being dispensed through the tube. Owner primed the pump and worked the air out, sanitizer was eventually detected at the correct concentration. Corrected on site.
- Handwashing signage
Observation:There is no handwashing sign at pizza kitchen handsink, designating which of the two compartments is for handwashing only. A sign was printed, corrected on site.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:There is no thermometer in the glass-door cooler in pizza kitchen.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation:Several pans of food in walk-in cooler from previous meals were not labeled with date of preparation or use-by date.
- Test kit provided and used to measure sanitizing solution concentration
Observation:Test papers in use at the establishment expired in October and November 2015, and are no longer able to accurately measure sanitizer concentration.

- Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours
Observed gravy in deep, covered pan in walk-in cooler at 53 degrees, whole turkey breasts in covered pan in walk-in cooler were 49 degrees. Both were prepared last night, did not reach 41 degrees within 6 hours, discarded, corrected.

Observed ice scoop stored on top of ice machine upstairs - surface is soiled.
