Observation: No screen installed on the window with the ventilator in the kitchen and when ventilator is off, the insect can come to the kitchen trough the small wholes. The openings shall be protected against the entry of insects and rodents by: installing the (1) 16 mesh to 25.4 mm (16 mesh to 1 inch) screens, (2) Properly designed and installed air curtains to control flying insects, or (3) Other effective means. Install the screen.

- Cleanability of floors, walls, and ceilings
Observation: Wooden shelves in dry storage are chipped and peeling away. All surfaces exposed to food must be easily cleanable, smooth and non-absorbent. Repaint with the 2 coats light color paint.
- Toilet room accessible to employees
Observation: Employee restrooms are not accesable to the employees. Toilete rooms must be conveniently located and accessible to food employees during all hours of operation.
- Storage and maintenance of wet and dry wiping cloths
Observation: One wet wiping cloth not held in sanitizer. Wiping cloths used to clean food contact surfaces shall be maintained in sanitizer. Manager corrected on site.
- Posting of a valid license
Observation: Most recent inspection report posted/displayed in the kitchen's board. Must be posted in visable level to the public, for example in the cafteria.
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Observation: Ventilation system in restroom (RR) is not working. Repair or fix it to ensure area is ventilated properly for cleaning and replacement if not designed to be cleaned in place.

Observation: Most recent license posted/displayed in the kitchen on board. The most recent license must be posted in visible level to the public, for example on board in the cafeteria.

Observation: Employee RR does not have working ventilation system. All RR must have functioning mechanical ventilation system. Repair as needed

Observation: Shelving in dry storage is chipped and worn. All surfaces exposed to food must be easily cleanable and non absorbent. Paint with 2 coats light colored paint.

- Light bulbs, protective shielding
One of the light bulbs on hood system were not shielded. Must shielded , coated, or otherwise shatter-resistant in area where there is exposed food.

Observed two dented cans. Food packages must be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.

Employee restroom does not have a functioning vent fan. It is necessary to keep rooms free of excessive heat, steam, condensation and obnoxious odors, mechanical ventilation shall be provided. Repair. The manager reports that they don't use this restroom anymore.

Found (1) burned out light bulb in 3 compartment reach-in cooler. Provide a new light bulb to improve light intensity.

Employee RR does not have a functioning vent fan. It is necessary to keep rooms free of excessive heat, steam, condensation and obnoxious odors, mechanical ventilation shall be provided. (Repeat violation). Repair as needed.
- Food storage, prohibited areas
Observed ice build-up in the Amana upright freezer closest to 3 compartment sink area. Defrost to prevent ice build-up and prevent contamination.
