- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation: Parmesan cheese found on a bar counter with the other spices and date 7/10/2015. Ready-to-eat, potentially hazardous food (Time/Temperature Control for Safety Food) must be refrigerated after opening at temperature 41* or below. Discarded voluntarily. Corrected on site. --Observation: A jar of salsa sauce found in walk-in cooler with no date marked. Food that is in a container or package must bear a date or day. Manger stated that is personal item. and will remove it.Corrected on site.
- Plumbing system maintained in good repair
Observation: Drain lines for ice machine has slight leak on the floor in the bar area. All plumbing system must be maintained in good repair. Fix the leak.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Test strips expired and showed sanitizer concentration low. Inspector used valid test strips and concentration was corrected between 50-100ppm. Must have valid test strips available to ensure proper concentration of chlorine sanitizer between 50-100 ppm.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: 1) Ceiling, on walk-in cooler have some buildup and dust that contaminate food. Clean and maintain cleaning at frequency to prevent future build-up. --Observation: 2) Pop corn machine has build up.The Food contact surface of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. Clean as often as necessary to keep it clean on sight and touch.

Observation: A small reach in freezer at the back has torn gaskets. Must replace gasket to maintain proper seal. This is repeat violation.

Observation: The ice machine has build up and shall be cleaned as often as necessary to keep clean
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: The ventilation system in the front restrooms are dirty. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt and other materials. REPEAT VIOLATION.
- Indoor and outdoor surfaces
Observation: 1)The front women's restroom has soiled or water damaged ceiling tiles. REPEAT VIOLATION. REPAIR AS NEEDED.
- Plumbing system maintained in good repair
Observation: 1)The bar water heater is non-fuctional and not reaching required water temperature. Owner stated the water heater was purchased 1 month ago and during this inspection called for service. The pilot light was also lit and the water was re-checked and hot water is available at this time of inspection. 2) The back restroom near the ice machine has an -excessive leak from the water faucet and flooded the floor during this inspection. The bar establishment has 4 restrooms available. Shall have a licensed plumber repair the back restroom and use the other 3 working restrooms. The owner during this inspection called Master Plumbing to repair back restroom handwashing sink faucet.
- Handwashing signage
Observation: The front women's restroom does not have a sign posted when food employees must wash their hands. All handwashing sinks used by food employees shall have a sign posted when to wash their hands. Given at this time of inspection. CORRECTED ON SITE.
- Drying mops
Observation: The mop was drying improperly and staff was educated how to air dry wet mops to prevent contamination.

Observation: The small reach in pizza freezer has torn gaskets. REPEAT VIOLATION.
- Smoke free air act
Observation: The back entrance door does not have a no smoking sign posted for consumers to view. Given the official no smoking sign during this inspection to post for consumers to view.
