Observation: Soiled utensils found in storage. Utensils were placed in the warewashing sink to be cleaned and sanitized.

Observation: Sliced tomatoes (top 57* and bottom 38*) and curry chicken salad (top 51* and bottom 36*) found with varying temperatures. Ice levels in the storage containers were low and not covering the food completely. More ice was added to the containers as well as water. Temperatures were taken again later in inspection and temperatures had dropped to below 41*. A new sandwich/salad prep table cooler is scheduled to be delivered and installed on 3/24/14.

Observation: Utensils and mixer attachments found in storage with cracks, chips, and flaking. New utensils and attachments have been ordered.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: Ambient air thermometers are not present in coolers. Thermometers are on order.

Observation: Employee observed not washing hands after leaving restroom and grabbing soiled dishware before returning to tasks in the kitchen. Management was notified at time of violation.

Observation: Food storage containers are not labeled with common name of food.

Observation: Wiping cloths used to wipe counter and equipment are not held in a sanitizer solution.
- Indoor and outdoor surfaces
Observation: Tiling under the warewashing sink is peeling and cracking off of the floor.
