Discarded tornados. KDA_3_60211C - --There was no list of donut names, ingredients, or allergen information on the bulk case of donuts located in the retail area. KDA_4_60111A - --Located in the pizza prep area, there was a dicer and a pizza cutter stored as clean that were soiled with dried food debris. COS- moved to wash area. KDA_4_60111C - --There was an accumulation of dust and debris along the inside rim of the Blue Bunny chest freezer near the pizza prep area. There is food debris on the bottom of the Blue Bunny chest freezer in the retail area. There is an accumulation of dust on the fan guards of the WIC. There is rust on the shelves of the WIC. There is rust on the outside of the ice machine located in the storage room. KDA_4_60211E4 - --There is mold on under the soda nozzles and on the ice chute of the Pepsi soda fountain in the retail area. KDA_4_90311A - --Located in the pizza prep area, there was a dicer, a box of individual pizza slice boxes, a sleeve of pizza boxes, and a sleeve of Styrofoam clam shell food containers stored on the floor. KDA_5_20315 - --There was a dual check valve but no intermediate vent on the soda fountain carbonator. KDA_5_20411A - --There was no hand sink located in the pizza prep area. The closest hand sink was located at the back of the store next to the soda fountain. KDA_6_20214 - --The doors to the men's and women's restrooms were not self closing. KDA_6_30112 - --There were no paper towels at the handwashing sink in the retail area. Corrected on site (COS)- provided paper towels. KDA_6_50111 - --There are several broken floor tiles located in the WIC. There is chipping paint on the outside of the cabinets located in the storage room. There is chipping paint on the wall next to the mop sink. KDA_6_50112A - --There is mold on the ceiling of the WIC. The cabinets under the three vat sink have an accumulation of debris on their bottom shelves. KDA_7_20111A - --Located in the storage room, there was a box of pot cleaner that was stored next to a container of saltine crackers. COS- moved.
