KDA_6_50111 - --There were approximately half a dozen ceiling tiles with water damage throughout the facility. There were approximately four missing floor tiles east of the warewashing sink. KDA_3_50116A1 - --Hot holding unit: sausage biscuit 127 F. The biscuit was reheated in the microwave then placed in the hot holding unit to hold one hour prior, as per employee. (COS-reheated to 194 F). KDA_3_50118A2 - --Three open packages of ready to eat taquitos were held in the walk in cooler six days with no consume by date label, as per manager. (COS-dated). KDA_4_60211E2 - --There was sludge and lime scale build up in the ice chute on the fountain machine. KDA_5_20212A - --The maximum water temperature at the hand washing sink in the men's room was 88 F. The maximum water temperature at the hand washing sink in the ladies restroom was 87 F.
