  • KDA_2_30114H - --In the Chinese deli, an employee was cutting ready to eat celery. Employee then touched sanitizing test strips, water hose connected to a faucet, then picked garbage up off the floor. Employee then took off gloves and put on new ones. Educated on hand washing. COS-employee took off gloves and washed hands before putting on new gloves and returning to cutting celery.
  • KDA_3_20215 - --On the shelves in the bakery, there were open packages of bread that were exposed. Person in charge said they will look into what they can do to fixed it.
  • KDA_3_30511A1 - --in the dairy walk in cooler, there was a container of wrapped packages of sliced cheeses that was sitting on the floor. COS-put on top of a box.
  • KDA_3_50116A1 - --cooked broccoli and squashed in the steam table was at 129.4F. It had been in the steam table since 10:00AM. COS-Voluntarily discarded.
  • KDA_3_50116A2 - --Sitting out on a make table in the Chinese deli, raw shrimp was 57F. Employee said it had been out for 10 minutes. COS-Employee put in cooler. Sitting in food prep sink in Chinese deli under running water was cream cheese at 70.3F, 74.8F. Employee said it had been thawing for half hour. COS-Employee removed it to cooler. In deli salad case: bacon pasta salad 44.9F, red potato salad 44F, cooked salmon 47.9F, cooked macaroni and cheese 46.9F, cooked tamales 50.3,52.1F, crab cakes 48.6F, egg plant parmesan 52.1F, cooked chicken breast 52.2F, baked beans 49.2F, pot pies 49.9F, real cooked mashed potatoes 48.6F, cooked ribs 52.3F, spaghetti salad 46.5F, coleslaw 44.1F, cooked general chicken 52.3F. Ambient of the south end deli was 52.3F on the risers. The ambient of the north end deli counter was 38.3F. All of these items were located at the south end. The log book showed at 2:00PM the bottles with thermometers were at 41F or below. COS-these items were voluntarily discarded.

Produce prep room, dairy walk in and meat prep room fans have accumulation of dust. The ceiling in produce prep room and the meat prep room have an accumulation of dust. The Chinese deli have stained ceiling tile.

  • KDA_6_50112A - --In the Chinese deli, there is water all over the floor. In the house ware Aisle, there is one old mouse dropping by the coffee pots. There are three old mouse droppings by the clear single glass ware. There are three old mouse droppings by the anchor 4 piece Reality 12 OZ. glass ware. 8 green droppings by Copco Lock and roll tumbler. Eight old droppings by the crock pots. There are mouse traps set under the shelves.

They are dating it with the slicing and packaging date not the original open date on the meat. Educated person in charge and employees on proper date marking. --In the deli counter: crab salad 42.6F. In the Chinese deli right make table, shredded cabbage 39.7F. Reach in, cooked shredded chicken 38.3F. In the lunch meat counter, sliced corned beef lunch meat 39.2F. --In the steam table: roast beef 134.7F, fried rice 136.8F. --The ware washing machine hot water sanitizing was 185F.

  • KDA_6_20215A3 - --In the vendor stock room, there is a 1" gap along the bottom of the left door on the east side. In the general merchandise stock room, there is a 1/4" gap along the bottom of the door on the north side.
  • KDA_6_50112A - --In the chip aisle, there is chip debris on the bottom shelf under the Kroger tortilla chips and Kroger pretzels. In the pet food aisle on an end cap, there is loose cat food under the shelf of 16lb bags of Pet Pride cat food. In the baking aisle, the following items had been chewed through, and had holes in them causing an excess of flour to be laying on the shelf and under the shelf. one, 4lb bag of Morena pure cane sugar, one, 10lb bag of Gold Medal flour, four, 5lb bags of Gold Medal flour and three, 10lbs bags of Kroger 10lbs of flour. . COS-person in charge clean the shelf and took the kick plate off and cleaned under the shelf. In the loose flour pile, there were about ten old mouse droppings. In the house ware aisle, the following area's had old mouse droppings: two old dropping behind the Cusinart 5 speed mixers. Two old droppings behind the kitchen aid blenders. 13 old droppings behind Pyrex mini spring form pans. One old mouse dropping behind green square plates. Two old mouse droppings behind black and decker coffee maker.
  • KDA_Defacto - --Licensed pest control has been out 8/13/15,8/12/15, 8/11/15. It was noted on pest control inspection report, there was no activity on those dates.
