KDA_4_60111A - --Stored as clean on shelving over 3 vat sink, 3 plastic containers with old date marking sticker residue on surface. One plastic container holding rolls on counter by steam table top with old date marking sticker residue on surface. Tomato slicer stored as clean on shelving for clean equipment where squeeze bottles are stored as clean with visible food debris on surface. Counter mounted can opener stored out for use with dried food debris on blade. COS rewashed. KDA_5_20515B - --Female toilet room is not working. KDA_6_501111A - --Approximately 20-25 live flying insects in establishment in warewashing area, food prep area in back of kitchen, sandwich prep area, as well as in dining area for consumers. PIC stated they have monthly pest control. KDA_7_20111A - --Working spray bottle of cleaner, per label, stored on lower shelf next to packages of RTE bread. Working spray bottle of cleaner, per label stored on shelf under prep area next to packages of gloves to be worn when handling food. COS moved. In back storage area, 4 containers of hand soap stored next to rolls of paper towels for use at handsinks. COS moved.
