Let her know that the open date, 8/7, counts as day one, and 8/13/15 is day two with five more days to use it in after this day. COS - she corrected the date marking. In the True cooler in the kitchen there is a bag of ready-to-eat meat balls dated 8/3. The manager said they were opened 8/3 and partitioned prior to being frozen. They were thawed on 8/17 per the manager. COS - she updated the date marking with a thaw date. In the cooler in the back there is a Ziploc bag of diced ready-to-eat ham that is dated 7/27 and a Ziploc bag of opened, ready-to-eat Sliced Italian Sausage that is dated 7/26. The manager said both of these items were removed from the freezer yesterday (8/17). COS - she updated the date marking.
- 4-102.11(B)(1) - --There is a head of cabbage being stored in a plastic Walmart grocery bag. The cabbage is in direct contact with the plastic of the bag. COS - the manager discarded this cabbage.
- KDA_2_30114I - --The manager handled a cell phone prior to grabbing a pair of gloves, putting them on, and taking a sandwich out of the oven and placing it onto a plate with her gloves hands and putting lettuce/tomato on the sandwich. Corrected on Site (COS) - Explained the hand washing violation to her. Did not require food to be discarded.
- KDA_3_10111 - --There is a cut head of purple cabbage in the cooler in the back. There is no date marking with this cabbage to indicate when it was originally cut. The manager is unsure when the cabbage was first cut. COS - the manager discarded the cabbage. In the pizza make table there is an opened bag of ready-to-eat corned beef that is not dated. The manager is unsure of the exact date. COS - she discarded it.
- KDA_3_30212 - --Throughout the kitchen there are unlabeled items that were identified as Mayo, Thousand Island Dressing, Cinnamon Sugar, and Flour.
- KDA_3_30412B - --There is a plastic cup in the flour being used as a scoop that has no handle or designated 'grab here' spot. The scoop is in direct contact with the flour.
- KDA_3_30611 - --At the salad bar the croutons, bacon bits, and sesame seeds are exposed without protection from the sneeze guard or other means. Recommending to manager that in the future these items be placed under the edge of the sneeze guard rather than just to the side of the buffet.
- KDA_3_50118A1 - --In the Pizza Make Table there is a bag of ready-to-eat sliced Italian Sausage that is dated 7/26 (opened and frozen), 8/5 (thawed). COS - the manager discarded the sausage for being held past 7 days. In the Pizza Make table there is a bag of opened, ready-to-eat salami that is dated opened/frozen 7/26. Thawed 8/4. COS - the manager discarded the salami for being held past 7 days.
- KDA_3_50118A2 - --In the True cooler in the kitchen there is an open gallon of milk with no date marking on the container. The manager said it was opened two days previously (8/16/15). COS - she dated the container.
- KDA_4_10111A - --There are onions being stored in a Sterilite container that has no markings indicating it is food grade. The onions are in direct contact with the container. There are croutons being stored in a Sterilite container with no markings indicating it is food grade. The croutons are in direct contact with the plastic of the container.
- KDA_4_20211A2 - --The lid to the dough container has gouges across the surface of the lid. The margarine container at the pizza make table has the center cut out of the lid to make it easy to brush butter from the container onto items. The ice scoop is cracked down the center of the scoop. The lid to the cinnamon sugar container is cracked. There is a cracked plastic food container above the 3 vat sink. There is a cracked container holding food in the freezer in the kitchen.
- KDA_4_50213A - --There is a single-use plastic cup being reused as a scoop in the flour. There are single-use plastic containers being reused to store salads for the salad bar.
- KDA_4_60111A - --There is a sterilite container on the floor storing onions. This container has sticker on the outside of the container that was identified as remaining from when the container was originally purchased. There are two plastic food containers above the 3 vat sink that an employee said were clean dishes. These containers have sticker residue remaining on the outside. In the freezer in the kitchen there are two containers storing food that have sticker residue on the outside of the container. The sticker residue was identified as remaining from when the container was originally purchased. There are two containers in the True Cooler in the kitchen that have sticker residue remaining from previous date marking on the containers. The croutons and bacon bits containers have sticker residue on the outside of the containers, identified as remaining from previous date markings stickers. There are lids on a shelf in the back storage room that are being stored as clean and have sticker residue remaining on the outside.
- KDA_4_60111C - --In the vent hood the hold for the vent that goes up into the ceiling has a buildup of dust in it. There is a box fan blowing in the kitchen that has an accumulation of dust on the cover.

In the waitressing/front area there is a bucket of sanitizer directly above napkins, utensils, and dessert toppings. COS - the manager rearranged. There is a container of white board cleaner/maintainer in a tub directly on top of the freezer in the kitchen.
