- KDA_3_50116A2 - --In walk in cooler - ambient temperature was 58F. Internal food temperatures were raw hamburger 54F, cut lettuce 57F, raw shelled eggs 55F, raw battered chicken 54F, queso 55F. Manager and owner did not have any recorded temperatures, and were unsure of how long the temperature had been above 41F. Corrected on site (COS) - all potentially hazardous foods were disposed of in dumpster behind establishment (container of raw battered chicken, container of raw battered fish, 35# raw hamburger, 3 bags of jalapeno queso cheese - marked 'keep refrigerated', container of cole slaw, 3 gallons of milk, package of sliced deli ham, package of sliced deli turkey, container of nacho cheese, 4 bags of cut lettuce salad mix, 3 bags of shredded cheese, container of diced tomatoes, case of raw shelled eggs). Repairman was called in and working on unit during inspection.
- KDA_3_60311B - --The menus did not have a consumer disclosure identifying items 'cooked to order', hamburgers, steaks, eggs. COS - labels were placed on menus.
- KDA_4_10111A - --In food prep area, flour was stored directly in two Sterlite plastic food containers that are not safe for direct food storage. COS - manager lined containers.
- KDA_4_30111 - --The make table cooler had an ambient temperature of 49.5F. There was no potentially hazardous foods being stored in make table. Manager stated they were not using it until it was working properly. The walk in cooler had an ambient temperature of 58F, not maintaining a temperature of 41F or less. COS - all potentially hazardous foods were disposed of and repairman was called in during inspection. Unit repaired and ambient temperature was 43.4F and in process of coming down further in temperature.
- KDA_4_50111A - --The floor of the beer walk in cooler sags (broken down) at the front of the cooler.
- KDA_4_50111B - --The door seals of reach in cooler (south) were loose and hanging along the bottom and sides of door. The bottom of the keg cooler and walk in cooler were rusted and corroaded not allowing the doors to seal tightly.
- KDA_4_60111C - --There is a build up of dirt and food residue in all coolers.
- KDA_6_20215A3 - --Back screen door has a 3 inch gap along the top.
- KDA_6_50112A - --There is a build up of dirt, grease, and litter along edges and under equipment throughout facility, as well as on light bulb shields in food prep area.
- KDA_Defacto - --Sanitize solution on warewashing machine tested at 100ppm chlorine.
