KDA_3_50118A1 - --In the cooler there was an opened package of ready-to-eat Jennie-O Beef that was dated as opened on 7/13/15. COS - the owner discarded this. KDA_3_50118A2 - --In the cooler there is an opened package of ready-to-eat ham with no date marking on it. The owner said this was opened on 8/3. COS - she dated the container. In the cooler there are open containers of ready-to-eat luxury loaf and bologna. The owner said these were both opened 7/31. COS - she dated the containers. KDA_3_50118A3 - --In the cooler there is an opened package of ready-to-eat turkey that is dated to discard on 8/12. The owner said the package was opened on 7/31. COS - she corrected the discard date to 8/6. KDA_4_50111A - --There is peeling paint from shelves on the retail floor. KDA_4_60111C - --There are bugs caught on the side of the freezer near the prep area. (Note - this freezer is not in use and is going to be cleaned up after being brought inside.) KDA_5_50117 - --The unisex employee's restroom does not have a trashcan with a lid. KDA_6_20215A3 - --The back door to the establishment has a 1/4 to 1/2 inch gap along the bottom threshold. They are keeping the hose hooked up out of the sink.
