KDA_5_20515B - --The mop sink faucet hot water side leaks when the water is turned on. KDA_6_50112A - --The floor areas in the back storage room, under the syrup shelving and around the hot water tank have an accumulation of dead bugs and black debris. The light fixture directly above the cappuccino machine has an accumulation of lent debris. The ceiling, wall, ceiling vent and fly trap near the pizza oven all have an accumulation of lent debris. KDA_7_20111B - --Located in the cabinet below the hot food station there was one bottle of grill cleaner sitting on the shelf above 1 box of single use foil wraps for food items. COS. Chemical was moved. Located at the 3vat sink there was one bottle of dish soap, 1 bottle of multi purpose cleaner and 1 box of sanitizer tablets sitting on the shelf above the sinks. COS. Chemicals were moved. KDA_7_30111B - --Located on the retail shelving there were 5 bottles of hand soap, 1 bar of soap, 3 packages of wet wipes sitting on the shelf above 2 packages of single use napkins and 3 packages of single use forks.
