KDA_3_50118A3 - --In the WIC a container of mixed meats (pork sausage and beef) was dated for longer than 7 days. It was date marked to thaw from 10/19 to 10/22 and to discard on 10/28. The meat temped at 35 F. The manager said the individual bags of meats were opened and mixed together this morning. COS, manager dated correctly. Educated on proper date marking times. PIC said the company now makes labels automatically. KDA_4_20211A2 - --There were 4 pizza covers that had large cracks in them making them difficult to clean. COS, VD. KDA_4_60111A - --Most of the food containers stored as clean have sticker residue on them and 3 food storage containers stored as clean have food debris left on them. COS, moved to be cleaned and sanitized. The can opener had dried food debris on the blade. COS, moved to be cleaned and sanitized. Approximately 5 pizza guards hanging on the wall next to the MT had marinara sauce on them. PIC said they have not been used today. COS, cleaned and sanitized. PIC educated about washing equipment used for PHF's to be cleaned and sanitized every 4 hours. KDA_4_60111C - --There is dried food debris on the rack where the clean pizza covers are stored as clean. There is food debris in the nooks for the pizza pans and boards next below the pizza cutting station. KDA_4_60213 - --There is an accumulation of grease on the back of the fryers. KDA_4_90311B - --Food storage containers stored as clean on a rack near the mop sink were stacked wet. Educated PIC on proper drying practices. KDA_5_20213 - --The sprayer next to the warewashing machine is not at least 1 inch or twice the diameter of the water supply inlet to prevent contamination of the water supply. COS, sprayer hung up and PIC educated and I suggested she get a spring that permanently holds the sprayer higher. KDA_6_30114 - --The men's restroom does not have a sign to notify employees to wash their hands. Inspector left sticker. KDA_6_50111 - --There is a hole in the wall behind the mop sink. KDA_6_50112A - --There is food debris on the floors and walls in the warewashing area. There is a black residue on the floor surrounding the water heater. KDA_Defacto - --Discussed proper cooling procedures with PIC. She said she cools the sauce by taking the container out of the steam holder to allow it to cool. She then puts the sauce in a poly container and puts it in the WIC. Educated about proper cooling times, temperatures, and methods. --Note: chlorine test strips provided. --Note: On the rack for clean food storage containers there were 2 non-food grade Sterilite containers. PIC said she uses the containers for Jello. Educated her about using food grade containers for food. --Note: There are no raw meats on site. --Note: Discussed location of sanitizer bucket and spray bottles of cleaner being stored in close proximity to the box of mints below the register. There is a space between them so there is no violation. --Proper cold holding temperatures: MT: cheese 43 F, ham 41 F, lettuce 43 F, sliced tomatoes 40 F RIC below MT: meat sauce 42 F WIC: potato salad 41 F Salad bar: eggs 40 F, lettuce 39 F Delfield RIF: foods frozen WIF: foods frozen --Proper reheating procedures: Reheated sauces are put in the oven for reheating and then put in the sauce warmer for hot holding. --Proper RIC temperature: The RIC by the register for pizzas only: ambient 34.4 F --Proper sanitizer concentrations: Warewashing machine: chlorine concentration: 150ppm 2 sanitizer buckets: chlorine concentration 200ppm --Proper TPHC for buffet: Times marked for each pizza on the buffet.	10/20/2015	11	Regular	In
