KDA_4_20211A2 - --Five bowls stored as clean in the kitchen were chipped and broken along the top edge. COS: Bowls discarded. KDA_4_50111B - --Vent cover in WIC is hanging down. A bar covering a shelf in the door of the RIC is broken. A drawer containing food in the RIC is cracked and broken. KDA_4_60111A - --Meat slicer stored as clean was soiled with food debris on the meat gripping surface and around the handle. Corrected on site (COS): Person in charge (PIC) showed employee how to correctly disassemble slicer KDA_4_60213 - --Doors and shelves of RIC and reach in freezers are soiled with a buildup of dirt and grease. KDA_6_30411 - --The establishment is smoky with obnoxious fumes. There is no hood vent in the establishment. KDA_6_50112A - --Walls and ceiling tiles in the kitchen are soiled with dirt and grease buildup. There is a buildup of apparent mold and dirt around and in the drain by the ice bin. There are approximately 15 mouse droppings along one wall in the storage room. KDA_7_20111A - --On storage shelves next to ice bin, an ice scoop was stored in contact with lighter fluid. PIC said ice scoop was not used, and there was another ice scoop properly stored in the ice bin. COS: PIC moved unused ice scoop. Disinfectant wipes were stored in contact with pretzel mix and waxed paper. COS: Wipes moved to chemical area. KDA_7_20111B - --Lighter fluid was stored on top of ice bin. COS: lighter fluid moved to storage shelf. KDA_7_20911 - --Employee cigarettes were stored over single-use plates in the kitchen. COS: Employee moved cigarettes. KDA_Defacto - --All potentially hazardous foods in freezers were frozen. --Approximate concentration of quaternary sanitizer solution at three compartment sink was 200 parts per million (ppm), and at bar three vat sink was approximately 100ppm. --Discussed reheating food to 165F within two hours.
