KDA_3_50118A2 - --In walk in cooler, a container of RTE chicken did not have consume by date. Chicken prepared 11/06, per owner. COS - dated. KDA_3_50118A3 - --In walk in cooler, an opened gallon of milk was date marked 11/06 - 11/13, 8 days. COS - milk redated with consume by date of 11/12, educated. KDA_4_10119 - --The floor of the walk in cooler, has several floor tiles missing with raw wood exposed. KDA_4_60111C - --There is a build up of moldy food debris along the floor of walk in cooler. There is a build up of grease and lint on the vent hood filters. There is a build up of dirt and dead insects on light shield inside of vent hood. There is a build up of grease and food debris along top and sides of stove top, flat top grill, and fryer. There is a build up of encrusted grease/food debris on the baskets of fryer. There is a build up of dust and debris on the tops of all food equipment in kitchen, bar, waitress station. KDA_4_60211E4 - --The ice machine (in bar area), had a build up of dust and dead insects along the top lip that is over the door of ice bin. KDA_5_20411A - --There is no hand sink available in the bar. NOTE: There is an additional basin to the side of the 3-vat sink, however, there is no water faucet. Owner placed soap & papertowels at sink and temporarily placed a container to hold hot water with a continuous running water for washing hands during operation, until a faucet can be installed. KDA_5_20511B - --In the kitchen hand sink, there were soiled pitchers and scrub pads. Corrected on site (COS) - items removed from handsink. KDA_5_20515B - --In front of the kitchen restroom doors, there are 2 pieces of raw plywood covering a large hole in the floor, approximately 5 ft by 6 ft. Owner stated the sewer line collapsed about 2 months ago, and they put in new line and sump pump. They have not filled in the hole since the repair. KDA_6_20211A - --The light bulb in the walk in cooler is not protected with shield. KDA_6_20215A3 - --The back door to the restaurant has a 1 inch gap along the top of the door. The entry door to the bar, has a 1 inch gap along the bottom of the door where the door sweep has worn away. KDA_6_30111 - --The mens restroom (restaurant area) did not have hand soap available. COS - soap placed in restroom. KDA_6_50111 - --At the front door, the carpet is torn, exposing the flooring underneath. There are some floor tiles missing in the kitchen area. KDA_6_50112A - --There is a build up of debris and dust along the floor edges in the kitchen. There is a build up of dust along ceiling tiles and air vents in kitchen area. KDA_7_10211 - --In the kitchen area, there were 4 unmarked spray bottles holding a yellow liquid. Owner stated liquid was 409 cleaner. COS - bottles marked. KDA_7_20211A - --In bar area, there was a can of Raid Ant & Roach spray, EPA 4822-447, and Raid Flying Insect spray, EPA 4822-569. Both sprays are not approved for use inside food establishments. COS - cans removed from bar. KDA_Defacto - --Cold Holding: Bar reach in cooler - 38.5F. Bar refrigerator - 41.1F. Kitchen walk in cooler - 41.3F ambient. Salad bar - potato salad 38F.
