- KDA_3_50118A1 - --In kitchen refrigerator, an opened package of sliced ham was date marked 01-07-16, held over 7 days. COS - ham disposed of in trash. In kitchen refrigerator, a container of cheese sauce (opened from original container) had a prep date of 01/04/16, held over 7 days. COS - cheese sauce disposed of in trash. REPEATED.
- KDA_4_60111B - --In kitchen, the fryer baskets had a build of up encrusted grease. REPEATED.
- KDA_4_60111C - --In kitchen, there is a build up of grease and lint on vent hood filters and on the floor and walls around and under fryer. REPEATED.
- KDA_6_50111 - --There is water damage on ceiling tiles in store room beside employee lounge (there is liquor bottles, fountain syrup and other food items stored on shelves in this room). There are several floor tiles broken or chipped in front bar, seating area. The linoleum in the kitchen/kitchen storage area has pulled away, exposing the backing of the linoleum, with gray tape securing the flooring in kitchen entry way. REPEATED.

- KDA_2_30114G - --Employee cracked 4 raw shelled eggs on the grill with bare hands, then proceeded to plate the order, handling RTE toast with her bare hands without properly washing her hands and donning gloves. COS - interrupted employee, she washed her hands and disposed of toast in the trash.
- KDA_3_30111B - --While plating a customer order, employee handled 2 slices of RTE toast with her bare hands. COS - interrupted employee, she disposed of toast in the trash, washed hands, donned gloves, and toasted more bread for order.
- KDA_3_30211A1b - --In refrigerator in kitchen area, a container of raw bacon and container of raw hamburger were stored above RTE tortillas and RTE deli ham. COS - containers moved to bottom shelf. In Refrigerator #4, RTE chicken wings were stored directly under a box of raw shelled eggs. COS - wings moved on shelf above eggs.
