  • 4-302.14 - --Establishment uses bleach for sanitizer in the kitchen area. The bar area uses a chlorine bleach tablet, however, the only test strips available were for quat ammonia sanitizers.
  • 3-501.17(B) - --In kitchen refrigerator, 2 containers of sliced deli ham had prep date of 11-12-15. Manager said this date was when ham was sliced and placed in freezer, and the ham was placed in cooler on 01/06/16. COS - employee updated date mark on containers of ham.
  • KDA_2_30114G - --Employee cracked 4 raw shelled eggs on the grill with bare hands, then proceeded to plate the order, handling RTE toast with her bare hands without properly washing her hands and donning gloves. COS - interrupted employee, she washed her hands and disposed of toast in the trash.
  • KDA_3_30111B - --While plating a customer order, employee handled 2 slices of RTE toast with her bare hands. COS - interrupted employee, she disposed of toast in the trash, washed hands, donned gloves, and toasted more bread for order.
  • KDA_3_30211A1b - --In refrigerator in kitchen area, a container of raw bacon and container of raw hamburger were stored above RTE tortillas and RTE deli ham. COS - containers moved to bottom shelf. In Refrigerator #4, RTE chicken wings were stored directly under a box of raw shelled eggs. COS - wings moved on shelf above eggs.

  • KDA_4_60111B - --In kitchen, the fryer baskets had a build up of encrusted grease.

  • KDA_6_20211A - --The light bulb in the walk in cooler was not protected with a shatter resistant shield.
