- The following listed violations were noted on the pre-licensing inspections dated 4/30/12 and 5/1/12. These violations were to be corrected by the thirty-day inspection which was conducted today. Correct these violations by the follow-up inspection which will be conducted during the week of July 2, 2012:
- 6.4 Provide base cove along the wall/hand sink juncture by the gyro unit, to provide a cleanable surface.
- *2.4 Denote foods on the menu that will be undercooked, to inform consumers of the increased risk of eating raw or under cooked food items.
- 6.2 Provide 50 foot-candles of lighting in food preparation areas and under the vent hood. Currently 35-42 foot-candles.
- 6.1 Provide a light shield or shatter-proof bulb on the light fixture above the rear storage rack, and equipment washing area to prevent contamination.
- 6.4 Provide easily cleanable cleaning tile in the waitress area, to provide a cleanable surface.
- *3701.21-25 Provide certificates of completion for SERV-SAFE manger certification course, or Level 1 certification, for at least on
- PIC per shift. Level one certification course is available at www.servsafe.com/starters or www.statefoodsafety.com
- Additional violations observed today:
- C/*4.5/4.6 Observed soiled spatula was stored between the wall and knife box, a soiled knife was stored in the knife box, and soiled tongs were stored on the handle of the oven. Food contact surface shall be washed, rinsed, and sanitized and shall be clean to sight and touch.
- *7.0 Observed an orange chemical in a Home Depot all purpose spray bottle was unlabeled. Properly label toxic chemicals to avoid confusion and misuse. PIC was asked to label this bottle.
- *7.1 Observed Windex was stored on the shelf with cooking spray, spices, and sauces, also Stainless steel cleaner was stored hanging on the food rack. Toxic chemicals shall be stored in a separate designated area from food and equipment. PIC was asked to relocate these chemicals.
- *3.4 Observed the following food items were lacking date marking such as but not limited to: lunch meats, chicken wings, gyro meat, tzatziki sauce. Foods shall be dated the day of preparation and shall be used or discarded in seven days. PIC was educated on date marking.
- *3.4 Observed the garlic butter was holding at 43°F, turkey 43°F, hamburgers 43°F in the reach-in unit. Foods shall be maintained at 41°F and below to limit the growth of organisms.
- C/*4.4 QAC sanitizer in the bar three vat sink contained 100ppm. Quaternary ammonium chloride (QAC) sanitizing solution shall be maintained 200-400ppm in order to effectively sanitize.
- *3.2 Observed raw shell eggs stored above tortillas. Different types of raw animal foods shall be stored separate from each other and other raw or ready-to-eat foods to prevent cross-contamination. PIC was asked to relocate these foods.
- 4.8 Observed the rubber spatulas were stored in the knife box with the handle downward. Store utensils in a manner to prevent contamination of the food contact surface.
- 2.4 Observed a little boy was in the kitchen. Unnecessary people are not permitted in the kitchen.
- 4.8 Observed a case of sweet potatoes were stored on the floor. Store the sweet potatoes six inches off the floor to facilitate cleaning and to prevent contamination.
- 4.4/6.4 Observed the following areas were uncleanable or damaged:
- - door seals on the cooks reach-in unit
- - base cove by the fryer
- - observed a large amount of ice accumulation in the upright cook's freezer which was covering the rear wall of the unit, this accumulation also contained what appeared to be a paper towel. Repair this area and defrost the ice accumulation
- - wall surfaces by the three vat and mop sink
- - door seal on the rear freezer unit, and four door refrigeration unit
- - waitress salad cooler. Repair these areas.
- 4.1 Observed no thermometer in the salad cooler, currently holding at 41°F and below. Provide this.
- 4.5 Clean the floor surface by the grease trap which is soiled.
- 6.2 Repair the non-functioning light fixtures in the rear storage equipment washing areas, to provide light requirements of OAC 3717.
- 3.2 Observed the ice scoops were stored with the handle touching the food. Store the ice scoop in the food with the handle extended outward.
- C/3.2 An opened can of Red Bull and juice was stored in plastic cups in the consumable ice. Foods are not permitted to be stored in ice for human consumption.
- 6.2 Provide hand washing signs in all the restrooms to inform employees where to wash their hands.
- 6.1 Eliminate the air gap under the rear door to prevent pest entry.
- Note: Observed the pasta salad contained a sell-by use-by date of 5/25/12. PIC was asked to discard this item.
