- *4.5/4.6 Observed the ice machine chute was soiled and contained a mold like growth. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
- *2.4/5.1 Observed a squeeze bottle for a condiment was stored in the hand washing sink. Keep the hand sink free from equipment and accessible to facilitate proper hand washing.
- 6.4 Rear storage area with single-use equipment and food items was lacking base cove. Provide lacking base cove in this area to provide a cleanable surface.
- 6.4 Observed a hat on the pop syrup boxes and employee body spray was stored on single-use equipment in the rear storage area. Employee personal items shall be stored in a separate designated area from food and equipment to prevent contamination.

- *3.1/6.3 Observed a can of Chicken of the Sea tuna was dented in the panel body and close to the side seam. Packages of food shall be in good condition to project the integrity of the contents and t prevent potential contaminants.
- Note: Rear storage area with single-use equipment was lacking base cove. Reportedly, this area is mainly swept with and occasional mopping. Recommend to provide base if food products are relocated in this area, and if mopping is the main cleaning method.
