  • *7.0 Spray bottle labeled bleach water, but contained an soap clear substance with 0ppm of chlorine sanitizer. Properly label this spray bottle with the name of the chemical.
  • *6.2/2.4 Provide paper towels at the restroom hand sink, to facilitate proper hand washing.
  • C/R/4.1 Walk-in cooler unit with milk, was holding at 41°F, and contained and inaccurate thermometer.
  • R/4.4 Repair the damaged door seals on the display doors of the walk-in cooler.
  • R/4.2 Provide a chlorine test kit to assure 50-200 ppm is maintained.
  • R/6.2 /Observed the light fixtures in the retail area, and above the three-vat sink were non-functioning. Repair these fixtures.
  • R/5.4 Keep the dumpster lids closed, and clean the garbage in the receptacle area to prevent pest entry.

  • 4.5 Clean the shelves in the walk-in cooler, and the walk-in condenser covers, which are soiled.
  • Notes: Observed 5 Special K protein chocolate bars dated 6/22/11, Hungry Jack Pancake mix was dated 4/16/11. Check and rotate stock regularly. Remove these items from the sales shelves.
  • Public restroom was closed due to a recent robbery at the operation.
  • Bunn coffee pot is no longer in use.

  • R/*7.1 Observed the Lysol and Wet Ones wipes were stored with plates and above cups, and a variety of air fresheners and detergents were stored with and/or above plates, cups, and napkins. Store toxic chemicals in a separate designated area from food and equipment.
  • *3.1 Observed a dented can of tuna. Packages of foods shall be in good condition to protect the integrity of the contents and to prevent potential contaminants.
  • C/R/4.1 Walk-in cooler unit with milk, and sandwiches, was holding at 34°F, and was lacking a thermometer. Thermometer was provided today.
  • R/4.4 Repair the damaged door seals on the display doors of the walk-in cooler.
  • R/4.2 Provide a chlorine test kit to assure 50-200 ppm is maintained.
  • R/6.2 /Observed the light fixtures in the retail area, and above the three-vat sink were non-functioning. Also the walk-in cooler contained 5-10 foot-candles of lighting.
  • 5.4 Keep the dumpster lids closed, and clean the garbage in the receptacle area to prevent pest entry.
