- 5.1 Observed the three-vat sink faucet was leaking. Repair this faucet.
- 4.8 Observed the pizza boxes were stored on the floor. Store pizza boxes six inches off the floor to facilitate cleaning and to prevent contamination.
- Operation added a new make table. Foods and refrigeration units were holding at 41°F and below. Thank you for your compliance with the OAC 3717.

- R/*3.4/4.4 Reach-in refrigeration unit contained an air temperature of 49°F on the bottom of the unit. Also salami was holding at
- 50°F, provolone cheese was holding at 49°F, chicken wings were holding at 54°F, ham was holding at 48°F, & chicken tenders was holding at 44°F. The temperature device in the unit was reading 48°F. Foods & refrigeration units shall be maintained at 41°F and below. Note: This unit was not properly holding temperature during the last standard inspection. Repair this unit &/or replace it. Contact this office with make and model numbers of new equipment. Foods above 45°F and above shall be discarded.
- 6.4 Repair the damaged grout by the rear door to provide a cleanable surface.
- 3.2 Provide a handled scoop for the oats in the rear storage area. Remove the cup.
- 6.4 Store employee apron in a separate designated area from pizza boxes, to prevent contamination.
- *3.1/6.3 Observed 4 cans of fully seasoned pizza sauce on the canned rack. Packages of food shall be in good condition to prevent contamination of the contents. Store damaged can goods in a separate designated area.
- 6.4 Repair the damaged wall surface under the restroom soap dispenser, to provide a cleanable surface.
