  • *3.1/6.3 Observed a can of pizza sauce was dented in the panel body of the can. Packages of food shall be in good condition to protect the integrity of the food and to prevent potential contaminants.
  • *7.0 Observed one unlabeled red spray bottle, and one clear chemical spray bottle which contained a label that was not legible.
  • Toxic chemicals shall be properly labeled with the containers contents to avoid confusion and misuse.
  • *3.4 Observed the bottom of the Sandwich reach-in unit was holding at 41°F and the meatballs, sausage pieces, and chicken pieces were holding at 43°F. Maintain foods and refrigeration temperatures at 41°F and below to limit the growth of organisms.
  • Make proper adjustments to this unit.
  • *3.4 Observed salami and pepperoni were lacking date marking. Properly date mark these foods once opened, use or discard them in seven calendar days.
  • R/6.4/4.5 The following areas were soiled:
  • R/- Ice accumulation in the walk-in freezer on the condenser line
  • - Ceiling tiles and vents throughout the Marketplace area were soiled, stained, or dusty.
  • Clean these areas.
  • R/6.1 Provide a light shield or shatter proof bulb on the light fixture in the donut case, to prevent contamination.
  • R/4.1/9.0 Remove the non-commercial White Westinghouse microwave from premises. Provide commercial grade equipment.
  • 4.4 Repair the damaged self closure on the Ruggles Ice Cream unit.
  • 3.2 Observed solo cups were stored in the shredded cheese. Provide a handled scoop for dispensing the cheese.
  • 6.4 Repair the damaged wall surface by the scale, to provide a cleanable surface.
  • 4.1 Provide a thin probe digital thermometer for obtaining temperatures for thin foods.
  • 5.1 Repair the leaky three vat and preparation sink faucets. Plumbing systems shall be maintained in a state of good repair.
  • 4.4 Replace the damaged/chipped utensil drying basket, and damaged strip on the interior of the sandwich unit.. Equipment shall be maintained in a state of good repair.
  • 4.8 Store the coffee filters in a covered container or in a cabinet under the counter to avoid consumer contamination.
  • 4.1 Provide new a QAC test kit, current test kit will not measure 150ppm for the current QAC sanitizer.

Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. Replace or resurface scored cutting board.
  • *2.3 Observed and employee open coffee beverage. Employees shall consume beverages from a closed container in kitchen areas or in a separate designated area from an open container.
  • R/6.4/4.5 The following areas were soiled:
  • R/- Ice accumulation in the walk-in freezer on the wall, floor, and condenser line
  • R/- Floor drain under the preparation sink
  • R/- Floor surface and floor/wall juncture under the ice machine, pop syrup rack, preparation sink, and three vat sink
  • R/- Wall by the Powerade drinks in the rear storage area
  • - Door seals on the reach-in units
  • - Ceiling surface in the area by the pizza oven is stained or soiled
  • - Floor surface under the walk-in cooler racks
  • - Light shield in the walk-in cooler
  • - Interior wall on bottom in the right corner of the tea machine
  • - Bottom of the donut case
  • Clean these areas.
  • R/4.1 Provide new thermometer in the open air sandwich case to assure 41°F and below is maintained. New thermometer was provided today.
  • 6.2 Repair the nonfunctioning light fixtures in the rear storage area by the walk-in freezer, to provide lighting as specified in OAC
  • 3717.
  • 3.5 Relocate price tags covering expiration, or sell-by use-by dates on retail products.
  • 6.1 Provide a light shield or shatter proof bulb on the light fixture in the donut case, to prevent contamination.

  • 3.5 Provide lacking ingredient labels on the Cole slaw, tuna salad, chicken salad, and egg salad sandwiches in the sandwich case.
  • Note: Observed Snack Pack chocolate pudding contained a sell-by or use by date of 10/17/10, and Delmonte fruit cocktail contained a sell-by use by date of 8/27/10.
