Downstairs Ice machine baffle was found to be heavily soiled with black and gray mold. -Corrected by turning off ice machine and only using upstairs machine. Clean and sanitize on a routine basis to ensure proper food safety.

Lemons were observed being cut with bare hand contact. Employees shall avoid direct contact (bared hands) with ready-to-eat food whenever possible and, to the extent possible.

Ice scoop in middle of room bar had handle in contact with ice. When storing scoop in ice, ensure handle always remains out of ice. Between uses during service, dispensing and incidental utensils shall be stored: in the food with handle extended out of the food, in running potable water dipper wells or clean and dry. Corrected by education, pulled handle from ice.

Women's bathroom has waste receptacles with out cover. Provide covered receptacles for women's bathrooms. Easily cleanable, covered receptacles shall be provided for waste materials in toilet facilities including vestibules.
- Food contact surfaces of equipment and utensils clean
Interior of soda nozzle guns has soil build up. Clean as often as needed to maintain clean. The food-contact surfaces of all cooking equipment shall be kept free of encrusted grease deposits and other accumulated soil. Corrected by bartender, pulled nozzle apart and cleaned.
- Floors properly constructed, clean, drained
Drain in middle of room bar is slightly soiled. Clean as often as needed to amintain clean. Floors, floor coverings, mats, and duckboard shall be maintained clean.

Spray bottle with glass cleaner was not labled. Lable all toxics and cleaning solutions. Containers of poisonous or toxic materials necessary for the operation and maintenance of the establishment shall be prominently and distinctly labeled in accordance with law. Sanitary waste containers with lids are missing from women's bathrooms. Provide sanitary waste receptacles with lids to dispose of women's sanitary waste. Easily cleanable waste receptacles shall be conveniently located near the hand washing facilities if disposable towels are used.

Walls in ice machine/storage room are not constructed of smooth, easily cleanable, non-absorbant surfaces. Install FRP Board as stated. Walls and ceilings of garbage or refuse storage rooms shall be constructed of easily cleanable, non-absorbent material.
