Sanitarian responded to RFS 14-0243 reported to the health dept on 6/3/14. Complainant reported that employee was not wearing gloves during food preparation. Upon inspection, workers were properly wearing gloves during food preparation. Manager Melissa reported that Burger King employees are required to wear gloves during all food prep. No further investigation is needed. MS/JD

6/9/2014Complaint Investigation
Site visit conducted due to complaint reported to the Health Dept on 4/29/14. Customer reported mold on his hamburger. During inspection, I spoke with manager Michelle who confirmed that a customer did report mold on his hamburger. She states that mold was not present on the patty rather it was a meat grisole from hamburger. She assured the customer that mold was not present and prepared the complainant another burger. She also offered him another meal free of charge to be used at a later time.  Upon inspection, there were no visible signs of mold on any of the frozen or cooked patties. MS

5/8/2014Complaint Investigation
HACCP: RFS 14-0114 was also addressed during routine inspection. Anonymous complainant reported that server was smoking cigarettes between bringing food to customer vehicles. Complainant was concerned about handwashing. During the inspection, correct handwashing procedures were observed of staff members. I instructed manager that smoking is prohibited during any food handling and food prep by all staff and is prohibited within 15 feet of the establishment. Staff shall wash their hands when returning from outside and when returning from their break periods. Kitchen staff was also informed of this.

Complaint was received that an employee took money then prepared food.

Discussed with the manager at the time, Zachary, who was informed of this complaint. Discussed hand washing and provided a hand washing haccp.

11/21/2013Complaint Investigation
  • Critical: Presence of insects/rodents. Animals prohibited
    Corrected: At this time did not observe any gnats or drain flies in the restrooms. Also at this time did not observe any flies in the kitchen area.
    Manager is using a fly spray to control flies. Discussed with the manager about coving food items and utensils when using the spray. Also, remember to wipe off counter tops and other items that may have came into contact with the fly spray.
    Maintain mesures to reduce, elimiate and contorl pest population. Keep the drive thru closed as often as possible.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized.

HACCP: Discussed glove use. Always wash your hands before applying gloves.

Sanitizer used is Chlorine.

All frozen food was frozen.

Observed proper time marking for using time as a public health control on food items. Reminded the manager to discard these products once the 4 hour time limited is reached.

All rechecks have a fee associated with them.
  • Food contact surfaces designed, constructed, maintained, installed, located.
    A to-go cup was observed to be in the sugar container under the front counter.
    Use handled scoops to dispense product and store them with the handle extended out of the product so that the handle is not touching the product.
    Corrected by removing the cup from the sugar.
  • Plumbing installed and maintained
    The urinal in the men's room was not flushing.
    Manager was instructed to put the urinal out of use until this can be fixed.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
  • Critical: Presence of insects/rodents. Animals prohibited
    1) Observed flies in the kitchen area. 2) Observed small gnats or drain flies in the men's restroom around the sink.
    Discussed with the manager who stated they use a fly spray at closing and in the morning before opening to help kill the flies. Reminded the manger to be aware of how the use the fly spray. Wash and sanitize all food contact areas that my have came in contact with the spray after applying the spray. Cover any exposed food or food product before spraying. Recommended contacting your pest control operator with alterative options for treating flies.
    A recheck will be conducted on or after 9/3/13.
  • Floors properly constructed, clean, drained
    The following are locations where the floor is soiled: Under the fryers, under the dry storage racks, under the equipment behind the front counter, under the smoothie/ice cream machines.
    Along with your daily cleaning routine also concentrate on the harder to reach areas which are long the walls edges under equipment and racks.
    Clean and maintain clean floors.
    Floors, floor coverings, mats, and duckboard shall be maintained clean.

Note: This inspection was conducted in conjunction with a complaint investigation. Complaint was that an employee was hanging out of the drive thru window beating a metal pan on the ground. The complainant felt this was unsanitary practices. The complainant also viewed an employee using their bare hands to assemble a sandwich.

Hand washing practices were discussed with the manager. The manager explained that the metal pan was not used for cooking but to sweep the trash, that the costumers throw out their car window on to the  driveway of the drive thru. The trash is swept to the side so items such as ketchup packs do not get ran over and splatter onto the side of the building. The manager was instructed to use other means to sweep up the trash. It is highly recommend that some type of trash can be used at some point in the drive thru lane for costumers to throw away their trash.
  • Food protection during storage, prepartion, display, service, transportation
    1) The cooked bacon on the shelf in the walk in cooler did not have a cover. 2) The bag of the frozen burgers in the walk in freezer was left open.
    Keep food protected to prevent contamination.
    Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
    Corrected: A cover was placed over the container of bacon and the bag of frozen burgers was closed up.
  • Critical: Hands washed, good hygienic practices (observed).
    Observed an employee switching tasked then applying gloves with out washing their hands.
    Corrected: Employee was instructed to wash their hands before applying gloves. Employee washed their hands.
    Hands shall be washed thoroughly before using utensils, including: deli tissues, spatulas, tongs or single-use gloves.
    Gloves used to handle ready-to-eat food, shall be single-use gloves (used for only one task).
  • Restrooms with self-closing doors, fixtures operate properly, facility clean, supplied with hand soap, disposable towels or hand drying devices, tissue
    The paper towel dispensor near the ice cream machine was no functioning.
    Corrected: The manager fixed the dispensor.
    Lavatories, soap dispensers, hand drying devices and all related fixtures shall be maintained in good repair.
  • Floors properly constructed, clean, drained
    The floor under the over and under the friers is soiled.
    Clean and maintain clean floors.
    Reviewed the cleaning schedule with the manager.
    Corrected: The manager swept under the cooking equipment and will have an employee deep clean at the end of the day.

.Complaint was for customer had soda nozzle in their drink cup.

Spoke to the manager at the facility. Manager stated a new employee was working that night and did not attach the nozzle correctly. Manager also stated these nozzles are hard to get back on correctly.

Detached nozzles to inspect for cleanliness. Nozzles were clean.
No violation noted during this evaluation.
