This is a change of ownership inspection for the establishment. Darrin Armstrong is the new owner. He was present at the time of inspection. There were no major violations found at the time of inspection.
Discussed maintaining a pest control operator and maintaining routine pest control operations.
There are many cosmetic type of violations that need to be corrected, these will be reviewed at the 30 day routine inspection.
Provide a self closing door for the hallway between the restroom and the back storage area.
- Food contact surfaces designed, constructed, maintained, installed, located.
There was excess ice build up in the pizza freezer. Routinely defrost the coolers to ensure that Equipment, utensils, and single-service articles shall not impart odors, color, or taste, nor contribute to the contamination of food.
- Non-food contact surfaces designed, constructed, maintained, installed, located.
1) HVAC pulling away from wall in storage room. 2) The vanity in the men's restroom in disrepair. 1) Reattach to the wall. 2) Repair/seal or provide a new vanity. Equipment, utensils, and single-service articles shall not impart odors, color, or taste, nor contribute to the contamination of food. Equipment shall be sealed to the adjoining equipment or adjacent walls or ceilings if exposed to seepage or splash.

There is no drain board on either side of the three compartment sink in the back storage room. Provide a cart or a separate area as there is shelving above the three compartment sink present. Drain boards and easily movable dish tables of adequate size shall be provided for proper handling of soiled utensils prior to washing and for clean utensils following sanitizing. Drain boards and easily movable dish tables shall be located so as not to interfere with the proper use of the dish washing facilities.
- Food contact surfaces of equipment and utensils clean
Interior of the ice machine soiled. Clean and sanitize all food contact surfaces and maintain clean.
- Non-food contact surfaces clean
Clean and sanitize all non food contact surfaces routinely.
- Plumbing installed and maintained
1) Ice machine leaking from the elbow at the back storage room. 2) There is no mop sink present, there is a dump drain. 3) The plumbing in the back storage room is slow to drain. 1)Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code. 2) Drain lines from equipment shall not discharge waste water in such a manner as will permit the flooding of floors or the flowing of water across working or walking areas or into difficult-to-clean areas, or otherwise create a nuisance. 3) At least one utility sink or curbed cleaning facility with a floor drain shall be provided and used for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water or similar liquid waste in new or extensively remodeled establishments.

There is no self closing mechanism in the men's restroom. Provide a self closing mechanism. Toilet rooms shall be completely enclosed and shall have tight fitting, self-closing doors. Toilet facilities, including vestibules, if present, shall be kept clean and in good repair and free of objectionable odors. - Floors properly constructed, clean, drained
Floors in hallway in disrepair. Floors in front area are chipping. Baseboards are in disrepair throughout. 1) Repair floors to ensure they are smooth, easily cleanable and non absorbent. 2) Re-epoxy or seal the floors properly. The juncture between walls and floors shall not present an open seam of more 1/32 inch.
- Walls, ceilings, and attached equipment, properly constructed, clean
1) Walk in cooler walls in disrepair. 2) Walls in storage room is in disrepair. 3) Walls in hallways in disrepair. 4) Ceiling tiles are in disrepair in restrooms and throughout. 5) Walls/vents throughout the establishment are soiled. 1) Provide FRP for the walk in. 2/3) Repair all walls. 4) Replace all ceiling tiles. Light fixtures, vent covers, wall-mounted fans, decorative materials and similar equipment attached to walls and ceilings shall maintained in good repair.
- Rooms and equipment - vented as required
There are no functioning vents present in the restrooms. Ventilation systems shall be installed and operated according to law and, when vented to the outside, not create an unsightly, harmful or unlawful discharge. Rooms shall have sufficient ventilation to maintain them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes.

There are many excess articles present in the back storage room and the front shuffle board area. Remove all unnecessary articles from the establishment. Only articles necessary for the operation and maintenance of the food service establishment shall be stored on the premises.
