Note:  Establishment is remodeling the top floor of the establishment.  It is currently not opened to the public.  All major renovations or major changes in food risk category require a Plan Review and approval before food service can begin. A Plan Review will be sent in the mail or can be obtained directly from our office at 2116 N Sheridan. Call 309-679-6161 with any questions regarding these requirements.
  • Restrooms with self-closing doors, fixtures operate properly, facility clean, supplied with hand soap, disposable towels or hand drying devices, tissue
    Men's restroom self-closing device is broken.  Restroom door propped open during inspection.  Provide a self-closing device to ensure restroom door is tight-fitting and closes when not in use.
    Toilet rooms shall be completely enclosed and shall have tight fitting, self-closing doors.

    No mechanical ventilation for restrooms.
