Items since preliminary inspection have been corrected: shelves have been sealed, coving has been added, and the gap under the kitchen door has been eliminated by replacing the entire door and frame.

This is a final inspection of the remodel/opening of the kitchen area.

Establishment has been given final approval to operate under the guidelines of Chapter 10, Food Safety, of the Peoria County Code.
No violation noted during this evaluation.

    There was no soap in the dispenser in the womens restroom. The paper towel dispenser is not available at the bar hand wash sink.

This is the final inspection for the establishment.  They will be utilizing bleach as a sanitizer.

Discussed providing routine pest control operations.

The establishment is not opening the rear kitchen. I did inform the owner that prior to utilizing the kitchen for cooking and even before beginning any remodeling or changing or adding of equipment, that a plan review and appropriate fee must be turned in and approved before anything is changed or added.  It is also important to note that the establishment must have certified personnel if they move to a medium or high risk.

There will be a pizza oven and table being placed near the window close to the hand wash sink.

There are minor items that require corrections, however, the establishment is approved to open and operate. License # 13-1160 was provided to the establishment.
  • Plumbing installed and maintained
    There is a leak from the mop sink faucet.
    Ensure all plumbing is properly installed and maintained.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
  • Restrooms with self-closing doors, fixtures operate properly, facility clean, supplied with hand soap, disposable towels or hand drying devices, tissue
    1) There are paper towel dispensers, however they are not yet on the wall. 2) The restrooms do not have self closing mechanisms. 3) There are no waste receptacles at the hand wash sinks.
    1 & 3) Ensure all sinks have soap, paper towels and a waste receptacle.  2) Attach the springs/hinge closers to the restroom doors.
  • Containers covered, adequate number, insect and rodent proof, emptied at proper intervals, clean
    There is accumulated boxes and trash at the outside back 'lot' area and there is no dumpster yet provided.
    Contact PDC or other appropriate disposal company.
    A sufficient number of garbage and refuse containers shall be provided to contain all of the waste that accumulates.

    1) The establishment was in the process of placing the self closing mechanism on the front door. 2) The side exterior exit door requires additional sealant/weather stripping to prevent entrance of pests, the inner door is functioning fine as a temporary 'barrier'.
    Provide a self closing mechanism and protect all entrances.
    Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight fitting self-closing doors, closed windows, screening, controlled air currents or other effective means.
  • Floors properly constructed, clean, drained
    The floors throughout are still in the process of being completed with wood and tile and carpeting(in main bar area).
    Floors and floor coverings of all food preparation, food storage, and utensil-washing areas, and the floors of all walk-in refrigerating units, dressing rooms, locker rooms, toilet rooms and vestibules shall be constructed of smooth, easily cleanable, durable material.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    The walls in the restroom need caulking along the plastic board and the painted areas. The door frames need to be installed and painted. Outlet covers need to be provided to all areas.
    Walls and ceilings, including doors, windows, and similar enclosures shall be maintained in good repair.
    Light fixtures, vent covers, wall-mounted fans, decorative materials and similar equipment attached to walls and ceilings shall be easily cleanable.
