There is a hook that an employee put up to hold the sprayer out of the flood rim. However, the sprayer is difficult to use when hooked and was found off of the hook. Devices to protect against backflow and backsiphonage shall be installed at all fixtures and equipment where an air gap of at least twice the diameter of the water inlet is not provided between the water outlet from the fixture and the fixture's flood-level rim. Correct by repairing the sprayer to where the sprayer no longer hangs below the flood rim. Discussed replacing the spring with the employee. 2nd re-check stemming from initial inspection conducted on 11/08/2013. At time of 11/08/2013 inspection, multiple live cockroaches were observed beneath dish machine in dish machine room. During 11/11/2013 first re-check inspection, 1 live cockroach was observed. Caulking had been added to holes in wall that were harboring pests. During 11/26/2013 second re-check, no live cockroaches were observed. Pest records were submitted which show that weekly pest service has been performed by a licensed Pest Control Operator. The last pest service took place on 11/20/2013 by Aardvark Pest & Termite Control, Inc. The Pest Control Operator noted that he observed no roaches during his inspection and "baited dishwashing area heavily." Continue weekly pest control service until Licensed Pest Control Operator has determined that issue is fully resolved and establishment can return to a monthly pest service schedule.

A licensed Pest Control Operator shall be employed to service the establishment on a weekly basis until the licensed Pest Control Operator has determined that the pest infestation has been fully minimized. A second re-check inspection will be conducted on or around 11/25/2013 to assess the effectiveness of pest control efforts as a result of moisture and harborage locations being decreased (floor drains are working properly and floors are maintained dry, as well as holes in wall having been sealed). If, during second re-check inspection, pest control records are provided which show weekly service and a visual inspection provides proof of minimization, the establishment will return to a routine inspection schedule and no further re-check inspections will be necessary. There are recheck fees associated with all rechecks.
