-Observed open drink containers on the floor in the walk in cooler along with other debris. Ensure floors are cleaned in 48 hrs; 04/04/15; to prevent the attraction of pests and vermin.

Notes: -Discussed employee hand washing policy and employee sick policy. Ensure employees sick with gastrointestinal symptoms ie. vomiting; diarrhea; jaundice; stay out of work at least 48 hrs after symptoms have stopped.-Handsink clean and stocked.-Restroom clean for employees only. Operator was using napkins for hand towels in the restroom; but said they had already ordered hand towels. Operator said the hand towels should be coming in tonights shipment. Discussed the importance of proper hand towels.-Ice from Arctic Glacier.-Walk in cooler 38F.-Reach in cooler 34F. All dates good.-Reach in freezer -19F. Ice cream freezer -15F.-Outside garbage stored properly.-Chemicals stored correctly.

Ok to issue permit to operate on the following conditions. 1. Remove particle board shleves inside walk-in cooler. Particle board is not an easily cleanable surface. All surfaces and materials must be smooth; easily cleanable; and non-absorbant. Wood is allowed as long as it has been sealed with light paint or polyurethane.2. Replace broken floor tiles by back door3. Provide weather stripping at back door to prevent rodent entry.-Discussed importance of handwashing-Discussed employee health. Ensure employees ill with gastrointestinal symptoms (vomiting and/or diarrhea) are excluded for at least 48 hours after symptoms stop.

Clean under three compartment sink/back corner4. Provide a "shatterproof" or "toughskin" type light bulb in back freezer to prevent glass breakage.5. Facility to be assigned a risk category 2. One certified food protection manager required. List of instructors given to operator.-Discussed importance of handwashing. Always ensure handsink is accessible.-Discussed employee health. Ensure employees ill with gastrointestinal symptoms (vomiting and/or diarrhea) are excluded for at least 48 hours after symptoms stop.
