Observed water leaking from above two shelving racks of the produce along the wall in the new warehouse; potentially from the roof but could not confirm. Need to move food items where water is leaking until leaking is stopped to prevent contamination.

The wood panels on the repack tables and above the stainless steel repack area are becoming worn with exposed wood. Need to sand and paint the wood areas so they are smooth; sealed and easily cleanable.

Observed mold growing in the ice machine; Need to deep clean; remove calcification and sanitize ice machine to prevent cross contamination. Recommend cleaning and sanitizing on a more routine basis. Ensure ice machine is draining properly to a floor drain to prevent water and mold accumulation.

No hand towels at the handsink in the repack area. Keep hand sinks stocked with soap and paper towels at all times to ensure proper hand washing.

At this time Bonanza Produces CFPM has expired. This facility will need to have an employee attend and complete a 16 hr Serv Safe course. Gave list of instructors to Robert Hernandez. This course must be completed with in 60 days of todays date. Once Serv Safe Course is completed come to Health District with Serv Safe certificate and test score attached to receive Washoe County Food Card. Complete Serv Safe Course and get Health District Food Card by 2/12/10.
