General Comments that relate to this Inspection Notes: PCO was verified by phone on 10/30/15; but the facility was unable to fax the invoice. An in store follow up was performed on 12/07/15. Invoices from Copesan and Advanced were obtained. Facility has fixed all doors as required by PCO. Rodent activity was observed by PCO during the November inspection. Facility must continue PCO service until no rodent activity is observed at which time the facility may reduce service. It is recommended to have PCO service on a routine basis to reduce the activity of pests and vermin. No signs of pests or vermin were noted during the reinspection.

[2] Toilet rooms: self closing doors; fixtures maintained; clean; hand soap/drying devices provided; proper waste receptacle -Observed broken soap dispenser in the mens restroom. Facility must repair soap dispenser or provide a pump bottle of soap within 24 hours; 10/28/15; to allow proper hand washing.
[4] Presence of vermin; outer openings protected; no prohibited animals -Observed mouse droppings in the back room and at the front of the store on the shelves storing dog food.*Facility must have a certified pest control operator maintain the store on a routine basis to reduce the number of pests and vermin and to keep the rodent population under control.*Facility must provide pest control operator invoice by 10/30/15.
