Remove/melt the ice; wash and sanitize the inside of the machine and do not put back into use until machine is clean.Violation must be corrected in 7 days; or by 2/19/2015Operator must remove ice from sales floor and discard. Do not put ice back on the sales floor until the machine has been properly cleaned and bags properly labelled.

Two glue traps (non-enclosed or self-contained) found on the sales floor with dead mice.Violation must be corrected in 48 hours or by 2/14/2015Operator must contact a pest control operator to develop a plan and address the vermin throughout the facility. Remove glue traps that could potentially be accessed by customers. Thoroughly clean and santize shelves; walls and floors with a bleach water solution to determine if the problem is ongoing.
