No violations observed at the time of inspection:Hot-holding checked good; trip-tip at 158F; hot-links at 156F; mac and cheese at 188F; bell peppers at 143F; pork at 141F.Cold-holding checked good; pico de gallo at 39F; cheese at 38F; raw chicken at 37F; tomatoes at 33F.Dishwasher ok at 50ppm chlorine; recommend new container of hypochlorite**** Handsink stocked and functional; observed handwashing. Observed gloves with ready-to-eat foods.Walk-in refrigeration ambient temperature at approximately 35F; food products properly stored. Recommend dates on all food products and please ensure a maximum hold time of (7) days for all potentially hazardous /TCS foods.Quatinary ammonia dispensing at 200-400ppm; sanitizer buckets available.Reviewed employee illness protocols; procedures ok.Ice machine observed clean.

The items noted on the inspection conducted on 11/21/2014 have been corrected.The following items must be corrected by 1/14/2015:1) Install atmospheric vaccum breaker on faucet on outlet for chemical dispenser.2) Install a mop sink outside by back door.Ok to use floor sink in kichen to dispose of water from mop bucket also.One CFPM is already on staff full time. Certificate #M140874 expires 7/16/2019. Keep Certificate posted in the establishment.Discussed employee health policy. Employees must be excluded from work if ill with vomiting or diarrhea until 40 hours after symptoms stop.

Ok to issue permit to operate.Bar glass washer operating at 122 F and 100 ppm chlorine concentration.The following items must be corrected prior to opening:Install closure trimming between top of bar wall and underside of bar counter along entire perimeter.Provide soap and paper towels for bar hand sink.Slide glass washer to left side to expose floor sink under glass washer.Clean and sanitize all work surfaces; food contact surfaces of equipment and utensils prior to opening.Provided copy of alohol sign; post and keep sign posted as required.Risk category 1 assigend. No Certified Food Protection Manager required for bar.Plans to open 11/29/2014. Hours will be 3 PM to about 10 PM Mon - Thu and 11:30 AM - 10 PM Fri - Sun

Ok to issue permit to operate.Walk-in cooler and refrigerated equipment at below 40 F.Noted hand sink stocked and functional.Dish washer at 110 F and 100 ppm Chlorine concentration. Ensure temperature rises to 120 F. Dish washer therrmometer not accurate (reading 186 F); calibrate or replace.Quat sanitizer in 3-compartment sink checked at 200 ppm concentration.Water temperature in adajcent 3-compartment sink measured at 124 F.The following items must be completed prior to opening:Install soap and paper towel dispensers in restrooms; install covered waste receptacles in women's restroom stalls.Cap off the floor sink inside walk-in cooler.Install door sweep on rear exterior door to seal gap under door.Provide quat test strips for monitoring of quat sanitizer as for proper concentration.Seal cabinet bottom around floor sink for soda dispenser.Clean and sanitize all work surfaces; food contact surfaces of equipment and utensils prior to start of food prep activitiesRisk category 3 assigned. Must have one Certified Food Protection Manager on staff full time who has completed an approved certification course by 1/21/2015Plans to open on 11/29/2014. Hours will be 3 PM to about 10 PM Mon - Thu and 11:30 AM - 10 PM Fri - Sun
