Observed a liquid with solids in an old crown royal bottle in the unplugged fridge without a label. Ensure all food items and drinks are properly labeled. Contact voluntarily discarded item. Corrected on site.

Reach-in fridge was unplugged. Observed bloody-mary mix in the fridge which states it must be refrigerated after opening. Contact plugged in the fridge and discarded the bloody-mary mix. Corrected on site.

Thermometers were not observed in either fridge. Thermometer missing in cold holding unit. Ensure all refrigeration and freezer units are stocked with a conspicuously placed thermometer in order to monitor unit temperature.

Facility is currently not using the Deli due to restrictions from the City of Sparks Environmental Control. Only the bar is in use. The risk level is being downgraded from a 2 to a1 due to lack of food service. Deli has had over 90 days to acquire a certified food protection. A Certified Food Protection Manager must be acquired by 10/07/12. This deli is assigned a risk category 2 health permit and a CFPM; certified food protection must be designated to this deli who is at least a 30 hour employee. A list of instructors was given to operator.

No food observed being prepared at time of inspection. Refer checked OK and noted at 40 F. All food served is prepackaged deli meats and potato salads and all sold on disposable dinnerware.
