Observed ice that had been bagged on-site without the facility name; address and contact information. This is a repeat violation from last year. All ice bagged at this facility needs contact information for tractability.

Observed ice scoop sitting on a plastic surface next to the ice machine. Store ice/food scoops within separate clean container or in product with handle up to minimize hand contact with product. This is a repeat violation from last year. Highly recommend cleaning and sanitizing ice scoop prior to next use to prevent contamination.

Test strips not available for ammonia sanitizer. Contact was able to demonstrate how he makes the sanitizer solution for cleaning coffee pots; surfaces and the ice scoops. Solution tested 200-300 ppm Quat. Reviewed contact time for sanitizer based on directions on the product label. Must provide accurate thermometers/chemical test kits.

Ok to issue permit to operate.The following items must be corrected by 10/28/2009:1. Install a reduced pressure backflow preventer between the soda carbonator and water supply line.2. Raise drain pipes of soda and ice dispoenser units to proivide them wih required air gap above the floor sink.3. Shelf inside cabinet where carbonator located is in disprepair. Replace shelf. wood must be sealed so that it is nonabsorbent.4. Install rabber base on wall adjacent to ice machine.5. Replace the burnt out light tubes of glass door freezer unit.6. Provide chlorine test strips to monitor for proper chlorine sanitizer concentration in utensil washing sink (50 to 100 ppm)7. Ice bagged on premises not labeled. Ice bags must be labeled with business information. Ice to be acquired from a provider from now on per operator.Risk category 1 assigned. No Certified Food Protection MAnager required for this facility.
