Notes:-No CFPM's on staff. This is a low risk permit where no food processing takes place and does not require a CFPM.-All items stoired at least 6" off the ground.-Bathrooms properly stocked.-Warehouse very clean.-Pest control operator is Terminex and come by on a monthly basis.-No food handling occurs just repacking of food suplements.-Chemicals properly stored.

Observed a hose from the janitorial sink faucet handing into the sink. There was no backflow prevention device on the faucet. In order to prevent the potential for backflow of dirty water from the sink back into the water supply; the hose must be disconnected when not in use or a vacuum breaker must be installed between the hose on the faucet. Corrected on-site.

OBSERVATIONS:- Reviewed last pest control report. No activity noted. Preventative treatment performed on a monthly basis.- All product (supplement pills; powders; etc.) was stored off the ground and away from all chemicals. All chemicals observed were properly stored and labeled. There is no repackaging of product at this facility. All product is containerized in bags or bottles that are sealed within boxes.- Employee restrooms were stocked and operational.

No violations noted at the time of inspection.OBSERVATIONS & RECOMMENDATIONS:- A minor amount of product is currently stored directly agains the walls in the warehouse. Recommend that over time and when possible a sanitary strip be placed (painted line) at least 18" from the wall and warehouse staff be directed to stock up to the line. This 18" clearance from all walls will make for ease of maintenance and cleaning activities. It will also help the pest control operator with treatment and preventative measures.- When not in use the hose on the mop sink faucet must be removed to prevent the possiblity of backflow of dirty mop water back into the water supply. Alternatively a vacuum breaker may be installed on the hose or faucet.- Supplement storage only. No liquid storage. Powders and capsules only. No refrigerated or frozen product at the time of inspection. Floors are cleaned by dry sweep method only.- Current PCO is The Bug Guy with once monthly treatments. No treatment reports available. Traps were observed during the inspection and no evidence of prohibited insects or rodents were observed during the inspection.- For recycling information you may contact Mr. Richard Sanchez @ 328-3743.
